As a courtesy, we will call your insurance company for benefit information, which is not a preauthorization or guarantee of payment. We advise each patient to call their insurance company for benefit coverage. If you desire, we can send in a predetermination for a written verification of approved services and the reimbursement amount. This process can take 4-6 weeks.
For your convenience, we accept Visa, MasterCard, American Express and Discover. For patients who wish to finance their treatments, we accept financing from CareCredit and Lending Club Financial, which offer interest free financing for those that are approved for credit.
We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect payments made by your insurance company during that time period. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. Refunds are processed weekly. Refunds under $5 are by request only.
Please remember that you are fully responsible for all fees charged by this office regardless of your insurance coverage.
